Information
The following materials shall be submitted when applying for change(s) of administrative license:
a.Application form
b.The original document (if registered) or mark (if filed) of the cosmetics administrative license
c.For the agency application, a copy of the authorization letter of the responsible entity for filing an administrative license in China and a copy of the business license shall be submitted and stamped with the official seal;
d.The documents that need to be submitted vary based on the change(s) that you want to make on the administrative license:
1)Product name change;
2)Changes on the name or address of the manufacturing company (including independent changes or mergers and acquisitions);
3)Change of the Chinese name of the manufacturer of imported products (the foreign name remains unchanged);
4)Changes on the responsible unit for administrative license in China;
5)Change on the actual manufacturing company;
6)Change on the category of special cosmetics.
